Head-to-head comparison

Semaglutide vs Tirzepatide: research comparison

Direct comparison of semaglutide (GLP-1 monoagonist) and tirzepatide (dual GIP/GLP-1) — receptor pharmacology, SURPASS-2 head-to-head data, weight-loss magnitudes, glycaemic effects, dosing kinetics and UK research framing.

Head-to-head trial: SURPASS-2

The single most-cited direct comparison between these two compounds is SURPASS-2 (Frías et al., NEJM 2021), which randomised 1,879 adults with type 2 diabetes to tirzepatide at 5, 10 or 15 mg weekly, or semaglutide at 1 mg weekly. At 40 weeks, mean HbA1c reductions were -2.01%, -2.24% and -2.30% for tirzepatide vs -1.86% for semaglutide; mean body-weight reductions were -7.6, -9.3 and -11.2 kg vs -5.7 kg. All tirzepatide doses were statistically superior to semaglutide on both endpoints.

The caveat: SURPASS-2 compared tirzepatide to semaglutide at the diabetes-licensed 1 mg weekly dose, not the obesity-licensed 2.4 mg weekly dose used in the STEP trials. The cross-trial comparison between STEP-1 (semaglutide 2.4 mg, ~15% weight reduction) and SURMOUNT-1 (tirzepatide 15 mg, ~22% weight reduction) is methodologically weaker but consistent with the SURPASS-2 superiority signal.

Mechanism: monoagonist vs dual agonist

Semaglutide is a pure GLP-1 receptor agonist with a 7-day half-life (the C-18 fatty diacid moiety gives strong albumin binding). Tirzepatide activates GLP-1 plus GIP. The therapeutic argument for adding GIP agonism is that GIP signalling potentiates insulin secretion and modulates adipocyte biology in ways that GLP-1 signalling alone does not — and the SURPASS-2 / SURMOUNT-1 magnitudes support this pre-clinical theory in human research.

UK regulatory context

Both compounds are MHRA-licensed prescription medicines:

  • Semaglutide: Ozempic (1 mg s.c. weekly, type 2 diabetes), Wegovy (2.4 mg s.c. weekly, weight management), Rybelsus (oral, type 2 diabetes).
  • Tirzepatide: Mounjaro (s.c. weekly, type 2 diabetes; weight management indication added 2024 in UK).

Compounded or research-grade material outside these MHRA-licensed pathways is not for human use and is supplied solely for laboratory research.

Side-effect profile comparison

GI side effects (nausea, diarrhoea, constipation, vomiting) are the dominant adverse event class for both compounds, occurring in 40-60% of trial participants depending on dose and titration schedule. Discontinuation rates for GI intolerance ranged 4-6% across SURPASS-2 and STEP-1 — broadly comparable between the two compounds at the tested doses. The SUSTAIN-6 trial of semaglutide and the SURPASS-CVOT trial of tirzepatide both monitored cardiovascular endpoints; semaglutide has the deeper cardiovascular outcomes file via the completed SELECT trial.
