Head-to-head comparison
Semaglutide vs Liraglutide: GLP-1 generation comparison
Semaglutide vs liraglutide — half-life, dosing frequency, weight-loss magnitudes from the STEP and SCALE trials, glycaemic effects in SUSTAIN/LEAD, and UK MHRA licensing differences.
Generational comparison
Liraglutide and semaglutide are both Novo Nordisk GLP-1 receptor agonists, but represent successive generations of the same molecular strategy. Liraglutide (approved 2010 in EU as Victoza; 2014 as Saxenda) uses a C-16 fatty acid for albumin binding, yielding a 13-hour half-life that requires daily subcutaneous administration. Semaglutide (2017 as Ozempic; 2021 as Wegovy) uses a C-18 fatty diacid plus a spacer, increasing albumin affinity enough to extend the half-life to approximately 7 days — enabling weekly dosing.
Head-to-head trial: STEP-8
STEP-8 (Rubino et al., JAMA 2022) randomised 338 adults with overweight or obesity to semaglutide 2.4 mg weekly or liraglutide 3.0 mg daily over 68 weeks. Mean weight reduction was -15.8% with semaglutide vs -6.4% with liraglutide. The trial provided the cleanest direct evidence that the half-life and receptor-saturation profile of weekly semaglutide produces a substantially larger metabolic effect than daily liraglutide at maximum approved doses.
UK MHRA licensing
- Liraglutide: Saxenda (3.0 mg daily, weight management), Victoza (1.8 mg daily, type 2 diabetes).
- Semaglutide: Wegovy (2.4 mg weekly, weight management), Ozempic (1 mg weekly, type 2 diabetes), Rybelsus (oral, type 2 diabetes).
Practical implications for UK research
For most metabolic-research contexts where direct comparison of incretin signalling is needed, semaglutide is the preferred reference compound — both for the magnitude of its effect and the depth of its modern evidence base (STEP-1 through 8, SUSTAIN-1 through 7, SELECT cardiovascular outcomes). Liraglutide retains research relevance as the first commercially-successful long-acting GLP-1 agonist and as a comparator in studies designed to isolate the effect of dosing frequency vs receptor binding profile.
