Phase 3 — Retatrutide

TRIUMPH programme: trial summary, limitations and UK relevance

Retatrutide's earlier phase 2 obesity trial produced one of the largest mean body-weight reductions reported in a phase 2 obesity study at the time of publication (NEJM 2023). The TRIUMPH phase 3 programme is evaluating whether the phase 2 signal translates into confirmed phase 3 efficacy and acceptable safety across obesity, T2D and MASLD populations. Phase 3 outcome confirmation, longer-term safety and regulatory review remain necessary.

Evidence: Phase 3

Key findings

Retatrutide's earlier phase 2 obesity trial produced one of the largest mean body-weight reductions reported in a phase 2 obesity study at the time of publication (NEJM 2023). The TRIUMPH phase 3 programme is evaluating whether the phase 2 signal translates into confirmed phase 3 efficacy and acceptable safety across obesity, T2D and MASLD populations. Phase 3 outcome confirmation, longer-term safety and regulatory review remain necessary.

Safety summary

Phase 2 data described GI adverse events at a magnitude consistent with the class. Phase 3 safety characterisation is ongoing; long-term cardiovascular, retinal, pancreatic and other organ-system signals require dedicated study.

Limitations

  • Many TRIUMPH studies still ongoing or with topline-only data at the time of review.
  • Retatrutide is investigational and not licensed for human therapeutic use anywhere at the time of review.
  • Cross-trial comparison with tirzepatide / semaglutide should not be interpreted as proof of superiority outside head-to-head studies.

UK relevance

Sets the trajectory for potential UK availability of triple-agonist incretin therapy; until phase 3 readout and regulatory review, retatrutide remains investigational in the UK.

What this evidence does not prove
  • Phase 2 weight-reduction magnitude does not confirm phase 3 efficacy.
  • Phase 3 efficacy does not establish cardiovascular, renal or long-term outcome benefit until specific outcome trials read out.
  • Retatrutide remains investigational and is not a licensed medicine.
