Outcome trial — Liraglutide (Victoza)
LEADER: trial summary, limitations and UK relevance
Liraglutide reduced the risk of MACE-3 compared with placebo in adults with type 2 diabetes at high cardiovascular risk over a median 3.8 years of follow-up. Reported hazard ratio: ~0.87 (95% CI ~0.78–0.97), with the effect concentrated in individuals with established cardiovascular disease at baseline (secondary-prevention subgroup). LEADER established the first robust cardiovascular benefit signal for a GLP-1 receptor agonist and, alongside SUSTAIN-6 and REWIND, anchored the class-level CV benefit narrative in T2D. Renal composite outcomes also improved in exploratory analyses. LEADER does not translate directly to non-diabetic populations, to obesity-only indications, or to newer molecules — the class signal is agent- and population-specific.

Safety summary
GI events were more frequent with liraglutide. Acute pancreatitis events were rare but the signal continues to be monitored.
Limitations
- T2D-only population; does not establish benefit in obesity without diabetes.
- Mechanism of CV benefit not fully delineated.
UK relevance
Long-standing UK-relevant evidence underpinning consideration of liraglutide (Victoza) in adults with T2D and CV risk.
- Does not establish CV benefit in obesity without diabetes.
- Does not establish equivalence between liraglutide and newer GLP-1 / dual / triple agonists.
