Half-life
Designed for once-daily oral dosing.
Category
Oral non-peptide GLP-1 receptor agonist (comparator)
Overview
Orforglipron is an investigational small-molecule GLP-1 receptor agonist designed for oral administration without the SNAC absorption enhancer required for Rybelsus. Unlike the peptide GLP-1 receptor agonists, orforglipron is a non-peptide molecule. Phase 2 data in obesity and T2D have been reported; the phase 3 ATTAIN (obesity) and ACHIEVE (T2D) programmes are in active enrolment / readout. Orforglipron is investigational in the UK.
Discovery & Research History
Orforglipron originated at Chugai and was licensed to Eli Lilly. Phase 2 obesity data (NEJM 2023) and phase 2 T2D data (Lancet 2023) demonstrated clinically meaningful effects. The phase 3 programme is the largest oral non-peptide GLP-1 RA programme to date.
Mechanism of Action
Small-molecule allosteric activation of the GLP-1 receptor. Because it is non-peptide, orforglipron is orally absorbable without the SNAC enhancer required for oral semaglutide (Rybelsus). The pharmacokinetic and food-interaction profile differs materially from peptide GLP-1 RAs.
Researched Effects
- Body-weight reduction in phase 2 obesity data.
- HbA1c reduction in phase 2 T2D data.
- Once-daily oral administration without specialised absorption-enhancer formulation.
Theoretical Research Protocols
- Dosage range
- Trial-arm doses are research records.
- Frequency
- Once daily oral administration in trial protocols.
- Duration
- Up to 36-week phase 2 reports; phase 3 programme durations 52-72 weeks.
- Administration routes
- Oral (clinical trial setting)
Safety Profile
Common side effects (research)
- Gastrointestinal events consistent with the GLP-1 class (nausea, vomiting, diarrhoea, constipation).
- Headache reported in some phase 2 cohorts.
Rare risks
- Long-term safety of small-molecule GLP-1 activation, including off-target effects, requires phase 3 characterisation.
Contraindications
- Investigational compound — eligibility per current trial protocol.
UK Regulatory Status
United Kingdom (MHRA): Investigational. Not MHRA-licensed.
European Union (EMA): Investigational.
Notes: Orforglipron is the most clinically advanced oral non-peptide GLP-1 RA at the time of review. UK regulatory pathway is yet to engage.

Frequently Asked Questions
Is orforglipron a peptide?
No. Orforglipron is a small-molecule non-peptide GLP-1 receptor agonist. It is listed here for cross-class context with the peptide GLP-1 RAs that dominate the existing licensed landscape.
Is orforglipron licensed in the UK?
No. Orforglipron is investigational across all major jurisdictions at the time of review.
How is orforglipron different from oral semaglutide (Rybelsus)?
Rybelsus is peptide semaglutide co-formulated with the SNAC absorption enhancer; absorption requires a fasted state. Orforglipron is a small-molecule agonist absorbable orally without an enhancer.
