Growth-hormone-axis combination research — study-design reference

Methodological study-design reference for combination research on the GH axis: GHRH analogues (tesamorelin, CJC-1295), GHS-R1a selective agonists (ipamorelin), and lipolytic peptide fragments. Framing is research-design literacy, not human-use protocol. URL retained for SEO continuity.

Mechanism rationale

Growth-hormone-axis combination research pairs a GHRH analogue — either tesamorelin (GHRH 1-44) or CJC-1295 — with the selective ghrelin-mimetic ipamorelin to model physiological pulsatile GH release rather than supplying exogenous hormone. Some research designs include AOD-9604 as a direct lipolytic-fragment arm to test whether C-terminal HGH activity contributes independently of the GH/IGF-1 axis.

Why pair GHRH analogues + GHS-R1a agonists

GHRH analogues stimulate somatotrophs via the GHRH receptor; ghrelin-mimetics (ipamorelin) act on the growth-hormone secretagogue receptor (GHS-R1a). The two pathways are synergistic in preclinical and small-cohort studies — they amplify each other's effect on pulsatile GH release without exceeding the physiological feedback envelope. This is the mechanistic argument underpinning the CJC-1295 + ipamorelin combination concept in the older research literature.

Reference research design

  • Primary mechanism arm: GHRH analogue + ghrelin-mimetic (CJC-1295 + ipamorelin OR tesamorelin alone).
  • Optional lipolytic arm: AOD-9604 in a parallel cohort to isolate fragment-only contribution.
  • Duration: 12-26 weeks (tesamorelin's pivotal VAT data was at 26 weeks).
  • Primary endpoint: visceral adipose tissue (VAT) volume by abdominal CT or MRI.
  • Secondary endpoints: IGF-1 (safety / efficacy biomarker), lipid panel, hepatic fat, glycaemic parameters.

Safety monitoring

IGF-1 is the critical safety biomarker for any GHRH-analogue research design. Tesamorelin trial protocols set IGF-1 monitoring at the upper limit of the age-stratified reference range; exceeding the +2 SD ceiling triggered dose reduction in the published programme. Glucose tolerance can deteriorate transiently under GH-axis activation — OGTT at baseline, 12 weeks and 26 weeks is the minimum for an adequately-monitored research design.

What this study-design reference does not do
  • It does not provide a protocol for combined human use of any research compounds.
  • It does not endorse human use of research-only growth-hormone-secretagogue compounds.
  • It does not provide dosing schedules, reconstitution maths or self-administration guidance.
  • It does not list, rank or recommend any supplier or sourcing route.
