Why this comparison is asymmetric

Semaglutide and AOD-9604 sit at opposite ends of the modern-peptide evidence spectrum. Semaglutide is among the most-cited drugs of the decade — Phase 3 evidence across obesity (STEP), type 2 diabetes (SUSTAIN), cardiovascular outcomes (SELECT, SUSTAIN-6), renal outcomes (FLOW), HFpEF (STEP-HFpEF) and ongoing programmes in NASH and neurodegeneration. AOD-9604's file stops at Phase 2b in obesity and has not been advanced to Phase 3. The pairing is rarely meaningful as a head-to-head efficacy contrast; instead, the comparison is useful as a framing exercise for thinking about what each compound is for.

Mechanism contrast

Semaglutide is an incretin-mimetic — a GLP-1 receptor agonist with a C-18 fatty diacid acyl chain that binds plasma albumin to extend the half-life to about seven days. Its effects are systemic and mechanistically diverse: appetite suppression via hypothalamic GLP-1R, glucose-dependent insulin secretion from pancreatic β-cells, delayed gastric emptying, and direct cardiovascular effects implied by SELECT outcomes that exceed what weight loss alone would predict.

AOD-9604 is a fragment, not an incretin. The 16-amino-acid C-terminal sequence (with an added tyrosine for stability) targets adipose tissue specifically — biasing adipocyte biology toward fatty-acid oxidation through β3-adrenergic-receptor activation and inhibition of acetyl-CoA carboxylase. The mechanism is narrower and, critically, sidesteps the GH/IGF-1 axis, which is the methodological reason to choose AOD-9604 in a research context: it isolates lipolysis without confounding by growth-hormone signalling.

Clinical evidence base

Semaglutide produced ~15% mean weight reduction over 68 weeks at the 2.4 mg dose in STEP-1; ~17% in STEP-3 (with intensive behavioural therapy); ~13.2% in STEP-2 with type 2 diabetes; and ~20% reduction in major adverse cardiovascular events in SELECT over 3.4 years. The trial programme is broad enough that semaglutide functions as the reference incretin in any modern metabolic-research design.

AOD-9604's Phase 2b programme produced placebo-adjusted weight reductions that did not reach significance, and the compound was not advanced to Phase 3. The published preclinical work is more positive — rodent lipolysis studies support the mechanism — but the clinical translation did not materialise. AOD-9604 remains a research compound used in adipose-tissue mechanism studies rather than as a weight-loss intervention.

Which to select for research

For research questions that concern clinical translation, population-level weight or glycaemic outcomes, or cardiovascular endpoints — semaglutide is the obvious reference compound. The trial record is the deepest in the metabolic-peptide class and the MHRA licensure provides the regulatory anchor for any work that needs to map onto licensed prescribing practice.

For research questions that concern adipose-tissue biology in isolation — the β3-adrenergic axis, lipolytic vs lipogenic balance, or GH-axis-independent fat metabolism — AOD-9604 is the more useful reference. The fragment's narrower mechanism makes it a cleaner probe for those research designs, even though its clinical efficacy file is thinner.
