Wegovy (semaglutide 2.4 mg) UK research guide 2026
Full UK research guide to Wegovy (semaglutide 2.4 mg): MHRA licence, STEP trial results, NHS availability, titration schedule, side effects, and comparison with Mounjaro and Saxenda.
What is Wegovy?
Wegovy is Novo Nordisk's proprietary brand of semaglutide at the 2.4 mg weekly maintenance dose, formulated specifically for chronic weight management. It received MHRA marketing authorisation in the UK and is the first GLP-1 receptor agonist licensed in Britain specifically for obesity rather than diabetes. Wegovy is distinct from Novo Nordisk's diabetes brand Ozempic despite containing the same active molecule; the higher dose and separate indication reflect the additional regulatory evidence package generated by the STEP clinical programme.
UK regulatory status
Wegovy holds a full MHRA licence for chronic weight management in adults with an initial BMI of 30 kg/m² or above, or 27 kg/m² or above in the presence of at least one weight-related comorbidity such as hypertension, type 2 diabetes, dyslipidaemia, or obstructive sleep apnoea. It is to be used as an adjunct to a reduced-calorie diet and increased physical activity. NICE appraisal TA875 recommends Wegovy for NHS specialist use in England. NHS supply operates through Tier 3 weight-management services; private prescription via licensed UK clinicians is available. The significant global supply disruption experienced between 2022 and 2024 — driven by the rapid uptake of semaglutide globally outpacing Novo Nordisk manufacturing capacity — had largely resolved by 2025 and supply in the UK was considered stable through into 2026.
Clinical evidence summary
The STEP programme (STEP-1 through 8) constitutes the pivotal evidence base for Wegovy. STEP-1 (n = 1,961 adults without diabetes) demonstrated mean weight reduction of 14.9% at 68 weeks versus 2.4% for placebo — a landmark result that established semaglutide as the most effective pharmacological weight-management agent at the time of publication. STEP-2 (adults with type 2 diabetes) showed 9.6% weight reduction at the 2.4 mg dose, consistent with the blunting effect of diabetes on GLP-1-mediated weight loss. STEP-3 combined semaglutide with intensive behavioural therapy and recorded 16% mean weight reduction. STEP-4 was a maintenance trial: participants who had lost weight on Wegovy and then switched to placebo regained approximately two-thirds of lost weight over 48 weeks, underlining the chronic nature of the treatment requirement. STEP-5 extended follow-up to 104 weeks, showing sustained weight reduction of 15.2%. STEP-7 examined once-daily oral semaglutide 50 mg (not Wegovy). The SELECT trial (2023) — though conducted using Ozempic-dose semaglutide rather than Wegovy — demonstrated a 20% reduction in major adverse cardiovascular events in people with overweight/obesity and established cardiovascular disease, providing the class with a cardiovascular outcomes indication.
Mechanism
Semaglutide is a selective GLP-1 receptor agonist. It delays gastric emptying, enhances glucose-dependent insulin secretion, suppresses glucagon, and acts centrally via hypothalamic GLP-1 receptors to reduce appetite and energy intake. The 2.4 mg dose used in Wegovy produces greater receptor occupancy and more pronounced central satiety signalling than the 1 mg dose in Ozempic. For the full mechanistic profile, see the semaglutide molecule page.
Dosing and administration
Wegovy is administered by subcutaneous injection once weekly, on the same day each week at any time irrespective of meals. The titration schedule is: 0.25 mg weekly for weeks 1–4; 0.5 mg for weeks 5–8; 1.0 mg for weeks 9–12; 1.7 mg for weeks 13–16; 2.4 mg from week 17 onwards as the maintenance dose. Each dose strength is supplied in a separate FlexTouch prefilled pen. Dose escalation can be deferred by four weeks if gastrointestinal tolerability warrants it. Injection sites are the abdomen, thigh, or upper arm.
Common side effects
Gastrointestinal effects are the predominant adverse events associated with Wegovy, as with the GLP-1 class generally. In STEP-1, nausea affected approximately 44% of participants on active treatment, diarrhoea 29.7%, vomiting 24.5%, and constipation 24.2%. The majority of gastrointestinal effects are transient and concentrate during dose escalation. Dose-dependent discontinuation rates due to adverse events were approximately 7.0% in STEP-1 at 2.4 mg, compared with 3.1% for placebo. Gallbladder events (cholelithiasis and cholecystitis) were also observed at higher rates than placebo — a finding consistent across the GLP-1 class and attributed to rapid weight loss effects on bile composition rather than direct drug toxicity.

Procurement and supplier position
This page is educational and does not advertise, sell, supply, recommend or rank any supplier, compounder, online pharmacy or telehealth service for Wegovy or any version of semaglutide. Wegovy is a UK prescription-only medicine and should only be accessed through a qualified UK prescriber following appropriate clinical assessment. Compounded or grey-market "semaglutide" is not the same regulated product as Wegovy. See UK POM context for GLP-1 medicines.
Frequently asked questions
- Is Wegovy available on the NHS in England?
- Yes. NICE technology appraisal TA875 recommended Wegovy (semaglutide 2.4 mg) for NHS use in England within specialist weight-management services for adults with a BMI ≥35 kg/m² and at least one weight-related comorbidity, or a BMI of 30–34.9 kg/m² in certain circumstances. NHS prescribing remains via Tier 3 services; direct GP prescribing is not currently commissioned.
- Was Wegovy affected by supply shortages in the UK?
- Yes. Novo Nordisk experienced significant global manufacturing capacity constraints between 2022 and 2024 that affected Wegovy supply in the UK and other markets. The MHRA issued guidance during this period advising prescribers to prioritise Ozempic (semaglutide 1 mg) for patients with type 2 diabetes. Supply normalised progressively through 2025 and into 2026 as Novo Nordisk expanded manufacturing capacity.
- How is Wegovy different from Ozempic?
- Both contain semaglutide, the same active molecule from Novo Nordisk, but they are licensed for different indications and at different doses. Ozempic delivers up to 1 mg (or 2 mg in some markets) weekly and is MHRA-licensed for type 2 diabetes only. Wegovy delivers up to 2.4 mg weekly and is licensed for chronic weight management. Using Ozempic off-label for weight loss is not endorsed and was a contributing factor to the supply crisis.
- What does Wegovy actually contain?
- Each Wegovy prefilled FlexTouch pen contains semaglutide — a GLP-1 analogue structurally modified at two positions to resist DPP-4 degradation, with a C18 fatty-diacid side chain attached via a linker enabling albumin binding and a half-life of approximately seven days. The 2.4 mg maintenance dose is approximately 2.4 times the highest approved diabetes dose of Ozempic (1 mg).
- Is Wegovy better than Mounjaro?
- Based on current trial data, Mounjaro (tirzepatide) demonstrates greater mean weight reduction at Phase 3. STEP-1 showed approximately 14.9% mean weight reduction with Wegovy 2.4 mg at 68 weeks, while SURMOUNT-5 (a direct head-to-head) showed tirzepatide 10 and 15 mg achieved approximately 47% greater relative weight reduction than semaglutide 2.4 mg at 72 weeks. However, individual response varies, and tolerability and cost factors influence clinical choice.
- Can Wegovy be prescribed privately in the UK?
- Yes. Licensed UK clinicians may prescribe Wegovy for weight management outside the NHS pathway via private weight-management clinics and online prescribing services. Private prescription does not require specialist referral but prescribers must satisfy themselves of the licensed indication and contraindications. Patients should ensure they are using genuine Novo Nordisk product obtained through the licensed supply chain.
